Tight Mitral Stenosis means that the Mitral valve orifice (between the leaflets) is narrowed to a very small area with a rigid margin.
By Severe Mitral Stenosis we refer to the circulatory embarassment that the Mitral Valve Stenosis is causing i.e. Back pressure of Blood on the pulmonary capillaries with pulmonary oedema (water in the lungs) and high pulmonary pressure. The right Ventricle may be under strain with Right ventricular failure etc.
